I agree to the terms and conditions of the club.
I will accompany my child during the practice, and I agree that I am responsible for accidents that may occur during participation (falls, contact with other participants or objects, weather effects such as intense heat and humidity, traffic, road conditions, accidents in the parking lot, and any other possible dangers). If there are any days when I cannot attend, I will inform the coach before the practice begins.
I agree that the current health condition of my child is good and that I will take full responsibility for my child’s health and participate in the practice.
I agree to receive the desired medical treatment if my child gets sick or injured while participating.
I agree that I will not be able to participate, if I take any action that significantly damages the image or rights of the club.
